SUMMARY CARE RECORD
Our surgery will soon be taking part in uploading patient summaries to the national spine. Information in your medical record - which will include your medication record, any allergies you may have and any recorded adverse reactions to medication - will be shared with authorised healthcare staff when they need to access this.
Please read details on this on what it means for you as a patient. Information can be found in the 'further information' section on this website. Or ask the reception team for the patient letter.
If you do not want your information shared, you must ensure that you 'opt out' by completing the 'opt out' form found on the website or at reception.
ON-LINE ACCESS TO BOOK YOUR APPOINTMENT & VIEW YOUR MEDICAL RECORD
From Monday 1st September 2014, 25% of routine GP appointments will be able to be booked on-line. You can book your appointments from 2 days and up to 28 days in advance. Please go to the 'Further Information' or 'appointments' section on this website for further details on how to 'register' for this facility.
You will also be able to register to view certain parts of your medical records on-line. By registering for this facility, you will be able to view your medication record, the immunisations that you have had along with any record of allergies/adverse reactions. This facility is only available to patients aged over 16 and registered with our Practice.
